ATTENTION- To all participants of the Bricklayers of Indiana Health and Welfare Plan: If you currently have a medical provider (doctor, hospital, clinic, etc.) that is out of network for UHC, please get us their names so that UHC can contact them and possibly get them to join the UHC network. The more info we can actually get will make it easier for them to cotact the providers- name, address, phone- type of specialty (for example OB-GYN, Oncology, Cardiology, etc.). But whatever information you can give us we will work with. E-mail or call your field rep. as soon as possible so that we can get this list to United Health Care.
